Placement Tool
This tool ties adult acute facilities to their long-term care and skilled-nursing partners in a manner that allows for real time evaluation of bed availability and matching to patient needs.
It searches for available beds based on many criteria including:
- Type of bed needed (e.g.; Clinitron, Bariatric, etc)
- Special care requirements (e.g.; Ostomy, Wound Care, Oxygen, etc)
- Type of payor accepted (e.g.; Medicare, Medicaid, etc)
- Cash requirements (e.g.; Medicaid +$10,000, etc)
- And many others.
It is designed to allow the case manager or social worker to sit at a patient's bed-side and based on a few point-n-click criteria, produce a list of facilities WHO CAN & WILL ACTUALLY TAKE THE PATIENT!
The tool allows hospitals to "invite" the organizations they work with to participate and will send an auto-generated request that links directly back to a profile page readily completed by each facility.
Once their profile has been established, sub-acute providers are reminded daily (if needed, by email) to update availability, and with a simple point-n-click can create an update.
This tool accomplished many extraordinary things for the average hospital:
- It saves thousands of hours per year of "submit & reject"
- It provides for better management of patient expectation
- It reduces length-of-stay by an average of 4 hours (overall reduction).
- It cuts faxing and calling back-and-forth down significantly
